- Atrophic Scars are shallow depressions that heal beneath the skin’s top layer. The most common cause is severe cystic acne. With atrophic scars, the skin fails to regenerate tissue, leaving an indentation.
Atrophic scars are further classified into 3 types:
- Boxcar scars: Broad, box-like depressions with distinct edges, caused by decreased collagen production during the healing stage.
Ice pick scars: Smaller, more narrow indentations in the skin that point downwards. Ice pick scars are notoriously difficult to treat and may require long-term, aggressive care.
Rolling scars: These scars vary in depth and have sloping edges that make the skin appear wavy and uneven.
- Hypertrophic scars: In contrast to atrophic scars, hypertrophic scars appear as raised lumps of scar tissue where the acne once was. This occurs when scar tissue accumulates.
- Post-inflammatory Hyperpigmentation: Trauma or inflammation activates melanocytes in the skin resulting in hyperpigmentation or “dark patches.” Hyperpigmentation can be problematic because it can take months to years for it to fully resolve. Picking on acne scars may increase the chances of developing hyperpigmentation.
